New SureForm Online Program

STEP 1 - Please print out these installation Instructions for reference

STEP 2 - Logging onto SureForm OnLine for the first time

1. To log in click: Sureform Online

IMPORTANT: When you enter your producer code, make sure you enter your complete producer code including the dash and two digit suffix (-xx) at the end.

When you log in for the first time, you must enter your zip code for the password.  After logging in you will be prompted for a new password. Enter a password you can easily remember, you will then be prompted to reenter your password for verification.

You will then be directed to the 'First Login' page. Follow the instructions to download the SureForm Online helper application.

STEP 3 - SureForm Setup

1. Agency Setup - Click on the Agency Setup tab. Your agency information should already be filled. Make any changes as required. Select the rater you are using from the dropdown.

2. Rater Setup.

WinFSC (AZ) - in WinFSC Tools -> Setup -> Bridges -> New:  Enter

       1. Bridge name: Sureformweb
       2. Supported products: check 'Auto'
       3. Path: c:\sureformweb\fscbrg.bat
       4. Set parameters to {Q}

ITC/Turborater (OK)  in ITC Tools -> Agency Management Setup -> New:

        1. System Name: Sureformweb
        2. Executable Name:  c:\sureformweb\turbobrg.bat
        3. Bridge Format: Turbo Tags Plus 1.0

AMS/QuoteWorks (AZ)  in Integration -> Enable SureForm

Click on 'Main' tab. The SureForm main screen is now displayed.
